The Brook Hospital offers highly structured Adult Chemical
Dependency Programs that utilize a Medical and 12 Step Model of
Treatment for treating chemically dependent adults (ages 18 and
over). The inpatient program is designed to provide the highest
level of direct patient care which includes physical, social,
spiritual, medical, psychotherapeutic and 12 step treatment
interventions. Following completion of the inpatient program, an
appropriate referral is then made for the patient to continue
treatment in the least restrictive level of care possible (i.e.,
partial hospitalization, intensive outpatient, outpatient,
aftercare program). The level of care recommended is determined by
the individual needs of the patient. All programs are individually
structured to focus on breaking the denial of Chemical Dependency
and beginning the process of recovery.
Levels of Care
Treatment & Discharge Planning
Before admission to any of the Adult Chemical Dependency
programs, patients receive a comprehensive assessment of their
mental, physical, and spiritual needs to determine the most
appropriate level of care. The treatment team, led by the attending
psychiatrist, develops an individual treatment plan for each
patient and meets regularly to review each patient’s progress and
to update treatment goals. Family members are also invited to
participate in treatment team and family sessions. The Brook
Hospitals recognize that recovery and healing is a lifelong process
and therefore discharge planning begins upon admission. The
treatment team works closely with each patient to set realistic
goals for continuing care following treatment and makes available
information or referral for further services as necessary.
